Most operators read the news report. I read the weather.
Sinclair Operations Group helps PE backed and founder led healthcare platforms see trouble in staffing, access, and revenue cycle weeks before it shows up in the P&L, then builds the teams and cadence to fix it.
The news report versus the weather report
Monthly financials, EBITDA, and survey results are the news report. They are accurate, they are important, and they arrive after the damage is done. By the time a margin miss shows up on the P&L, the cause is usually six to eight weeks old.
The weather report is the set of leading indicators your front line already produces every day: open shifts, unanswered calls, stalled authorizations, slow charge entry, and new hires who quietly stop showing up. Read them weekly, give each one an owner, and you get to act while the problem is still small.
Every engagement starts with a 30 minute Weather Check: a conversation about the signals you track, the ones you do not, and where the next storm is likely to come from.
The news report
- Monthly P&L and EBITDA
- Collections and AR aging
- Survey and audit findings
- Turnover reported at quarter end
- Patient complaints after they escalate
The weather report
- Open shifts for the next two weeks
- Call answer and lead to appointment rates
- Authorization and intake backlog
- Days from visit to claim
- Escalations per site and who resolved them
What the weather report has delivered
Client names are kept confidential. Results come from fractional, interim, and direct operating roles.
Where I help
I work as a fractional or interim COO beside a CEO, founder, or PE operating partner. The goal is always the same: leave behind a team and an operating rhythm that run without me.
Turnaround and stabilization
When staffing, access, or cash has slipped and the board wants a plan in weeks, not quarters.
- Staffing and shift coverage models
- Escalation paths and supervisor layers
- Billing lag and revenue cycle handoffs
De novo launches and acquisitions
22+ launches, joint ventures, and acquisitions across 5 states, including 5 dialysis clinics opened on $18M of capital.
- Launch playbooks and intake capacity
- Operational diligence and integration
- Licensure, credentialing, and survey readiness
Operating cadence
A weekly rhythm that turns data into decisions and gives every metric an owner.
- KPI scorecards and leading indicators
- EOS Level 10 meetings and OKRs
- SOPs that let site leaders solve problems themselves
Leadership bench
The work is not done until someone on your team can do my job.
- Manager development and succession
- Values based leadership training
- Automation that gives people time back

Fulcrum Leadership
A leader is the pivot point between two sides of an organization. Lean too far toward either side and the whole thing tips.
The brain side
- Data, KPIs, and scorecards
- Systems, SOPs, and automation
- Budgets, forecasts, and capacity plans
The heart side
- People, values, and respect
- Frontline realities in front of executives
- Coaching leaders to take my job one day
Bradley A. Sinclair Jr., MPA
I have spent 19 years running multi site, multi state healthcare operations: dialysis, ABA and behavioral health, urgent care, ambulatory surgery, ophthalmology, women's health, virtual care, and a 24/7 public health contract in New York City. Before founding Sinclair Operations Group in January 2020, I spent five years as Director of Operations at Fresenius Kidney Care, running 12 dialysis centers for 1,800 patients.
I describe myself as a human first. I lead with data and with respect, I give bad news together with a plan, and I measure my work by what keeps running after I leave.

I am open to the right permanent seat with a multi site platform. If that is your search, I would welcome the conversation.
